I get it neither from falling over nor the buckle rubbing on the Velcro, because it's been forever since the last time I fell and I never loosen the strap beyond where the Velcro touches the buckle.  I just slip my foot through the loop without ever fully undoing the strap.  The wear seems to be coming by constant but light rubbing against the bolt on the bar as the cuff tilts slightly with every step.  I've recently begun sticking the Velcro so it isn't tight against the bar, because the buckle holds all the force anyway.  I can't tell whether it's working or not because the damage is already so bad, but that's probably the problem.
